Required Forms

The following forms MUST be turned in to the Athletic Director (Mr. Faye) or Assistant Athletic Director (Mrs. Perez) BEFORE an athlete can attend tryouts:

  1. Physical Form (EL2) — This must be filled out by a physician, parent/guardian, and student.
  2. Intramural Consent and Release Form
  3. Extracurricular Activity Form — This form must be completed for EACH sport/club that a student is participating in. For example, if the student wants to play soccer and tennis, they must have one of these forms with Tennis next to Club, and another form that has Soccer next to Club.

Incomplete forms will be sent back home with the student. Mr. Faye or Mrs. Perez will meet with each student to let them know what they need to fill out if the forms are incomplete.

You can email your completed forms to Mr. Faye ([email protected]) or Mrs. Perez ([email protected]), or send them with your student-athlete to school.
